Some of our Arizona customers were recently notified by the United States Postal Service that effective July 1, 2009 their ZIP Codes beginning with "852" will change to "851. While we work to complete the programming changes that are required to make the new “851” ZIP Code work effectively with our automated systems, please continue to use your previous "852" ZIP Code to search for your regional premium rates, locate a network dentist or access the online enrollment application on this web site.
